By using our website or services, you agree to these Terms of Service. If you do not agree, please discontinue use of the site and services.
Rezolv Health offers telehealth medication-assisted treatment for addiction and mental health medication management. Services may include online assessments, video visits with licensed providers, prescriptions when appropriate, and follow-up medication management. Care is provided only in states where we are licensed.
Who Can Receive Care
To receive care, you must be at least 18 years old, be located in a state where we operate, and provide accurate health information. You also need a device that supports secure video visits.
Telehealth Services
Our services are delivered through telehealth. By using them, you consent to receive care through video and electronic communication. Telehealth can improve access but may have limits compared to in-person care. You may stop telehealth services at any time.
Medical Limits and Emergencies
Our services are not for emergencies. Call 911 for urgent medical issues. If you are in crisis, call or text 988. Completing an assessment does not guarantee a prescription. All clinical decisions are made by your provider.
Your Responsibilities as a Client
You agree to provide accurate information, attend scheduled visits, follow your treatment plan, and pay required fees. You are responsible for keeping your login secure and using services only for your own care.
Payment and Billing
If we accept your insurance, we will bill it on your behalf. You are responsible for any amounts not covered. Self-pay patients must pay disclosed fees at the time of service unless otherwise arranged.
Prescriptions
Medications are prescribed only when clinically appropriate. We may decline or stop prescriptions based on medical judgment. Some medications are not prescribed via telehealth. Prescriptions are sent to the pharmacy you choose.
Privacy and Confidentiality
Your use of our services is also governed by our Privacy Policy. Health information is protected under HIPAA and, when applicable, 42 CFR Part 2.
Website Use
All site content belongs to Rezolv Health. You may not copy, distribute, or misuse it. You agree not to misuse our services, provide false information, or attempt unauthorized access.
Limits on Liability
Rezolv Health is not responsible for indirect or consequential damages. Any liability is limited to the amount you paid for services in the prior 12 months, as allowed by law.
Changes to These Terms
We may update these terms from time to time. Continued use of our services after updates means you accept the revised terms.
